    This form should only be completed if you have written "Exempt" on line 4C of the Federal Tax Form opting to NOT have Federal Taxes withheld from your paycheck. Please note, this selection may be subject to tax penalties and fees based on your financial situation. Please consult with a tax professional or trusted advisor with any questions or concerns.
